PERSONAL RESPONSIBILITY FOR DATA QUALITY
I am responsible for following the six data quality standards as laid out in the Data Quality policy. The six data quality standards are:

VALIDITY
I will make sure data items held on trust computer systems are valid.

COMPLETENESS
I will make sure that mandatory data items within a data set will be completed. I will only use default codes where appropriate, and not instead of real data. If I need to go past a data item in order to admit that patient, the missing data should be reported for immediate follow up.

CONSISTENCY
I will make sure that data items will be entered into the system in the same way. Patients with multiple episodes will have dates entered in the same way.

COVERAGE
I will make sure that data will show all the work done by the Trust that I may reasonably be aware of or responsible for entering.

ACCURACY
I will make sure that data recorded on paper and on computer systems will accurately show what actually happened to a patient. I will make sure errors will be corrected as soon as possible and that hand written data is legible and is both signed and dated. At every appropriate opportunity I will check a patient's personal details with the patient themselves. I understand that unclear personal details may lead to important letters being lost, or the incorrect patient identified.

TIMELINESS
I am aware that if I record the data correctly it will help the treatment of the patient. I am aware that putting results of tests into the computer, or recording diagnosis and operations makes the information available to all treating the patient, even if they do not have access to the paper notes.
